What this form is for
Employer's Quarterly Federal Tax Return (941). The quarterly return on which an employer reports wages paid, tips reported, and the federal income tax, Social Security and Medicare withheld.
Deadline. The last day of the month after each quarter ends: 30 April, 31 July, 31 October and 31 January.

Most employers with payroll.
When you do not need it
Very small employers approved to file annually use Form 944 instead, and agricultural employers use Form 943. You cannot switch to 944 on your own — the IRS has to tell you to.
How to fill it in

What goes wrong
- Filing the return is separate from depositing the tax. Deposits run on their own schedule and being late on those is penalised independently.

Follow the address or e-file route printed in the current IRS instructions for this form. Those change between tax years, so check the instructions rather than reusing last year’s envelope.
