What this form is for
U.S. Passport Renewal Application by Mail (DS-82). Renews a US passport by post, without an appointment.
Who needs it
Adults whose most recent passport meets every renewal condition.
When you do not need it
You cannot use it if the passport was issued before you were 16, was issued more than 15 years ago, is damaged or missing, or is in a name you can no longer document. Any of those means DS-11 in person.
Before you start
Have these to hand — stopping halfway to look one of them up is how a ten-minute job becomes an evening.
- Your most recent passport, which is posted in with the form
- One 2×2 inch photograph
- A certified marriage certificate or court order if the name has changed
How to fill it in

What goes wrong
- Send it by a trackable method. The old passport travels with the application, and it is returned separately from the new one.
Where it goes
Submit it to the address or online portal named in the agency’s current instructions. Sending an application to a superseded address is one of the most common causes of delay.
