What this form is for
Request for Reconsideration (SSA-561-U2). Asks Social Security to look again at a decision you disagree with — the first step of the appeal process.
Deadline. Generally within 60 days of receiving the notice. Late requests need good cause.
Who needs it
Anyone who has received a determination they think is wrong.
How to fill it in

What goes wrong
- Say what you disagree with and why, specifically. "I disagree" without a reason gives the reviewer nothing new to work with, and the reconsideration reaches the same answer.
Where it goes
Submit it to the address or online portal named in the agency’s current instructions. Sending an application to a superseded address is one of the most common causes of delay.
