What this form is for

Application for a Social Security Card (SS-5). Applies for a Social Security card — a first card, a replacement, or a change of name.

Who needs it

US citizens and lawfully present non-citizens who need a number or a corrected card.

When you do not need it

A card you have simply mislaid is not worth replacing if you know the number and have used it recently — the card itself proves nothing beyond the number, and replacements are capped at three a year and ten in a lifetime.
